Officers in Nottinghamshire were first alerted to the driver as his black Ford Focus was using false registration plates, before it sped off down the wrong side of the A631.
The motorist was captured on dashcam careering along the dual carriageway from Gainsborough, Lincolnshire, and into Bassetlaw, Nottinghamshire, at around 6pm on October 15.
The suspect's car repeatedly weaved in and out of oncoming traffic in a frantic bid to evade capture.
He bolted away from the market town of Bawtry at speeds of over 80mph before hurtling through a red light.
As the car swerved back onto the right side of the road, he collided with another car before speeding away.
The video then shows officers finally managing to pull alongside the Ford and force it off the road near the village of Everton.
After the Ford crashed through a hedge and into a field, the driver then ran from the car with PC Haddon Smith in pursuit.

Footage released by Nottinghamshire Police then cuts to that officer's bodycam images, showing PC Smith pointing his Taser at the suspect and shouting: 'Give yourself up now. On the floor now, lay down.'
As he put the handcuffs on the man, PC Smith calmly tells him: 'You're under arrest mate.'
The 31-year-old man asks, 'what for?' as he is arrested on suspicion of dangerous driving, failing to stop, going equipped for theft, driving without insurance, and driving otherwise than in accordance with a licence.
He has subsequently been released on conditional bail.
PC Smith, of Nottinghamshire Police's roads policing team, said: 'As highlighted in this instance, we will not tolerate people who break the law by driving vehicles in a dangerous manner that puts themselves and other road users and members of the public at risk of serious injury or death.'
