The actress, 37, left little to the imagination as she showed off her bump in an edgy photoshoot that saw her cradling her stomach while covered in black paint.
Megan is already a mother-of-three - sharing sons Noah, 10, Bodhi, nine, and Journey, six, with ex-husband Brian Austin Green.
This will also be MGK's second child - having previously welcomed daughter Casie Colson Baker, 15, with ex Emma Cannon.
MGK and Megan's high-profile romance most recently made headlines after the A-list lovebirds called off their engagement amid cheating rumors, just months before the actress revealed she is expecting.
Megan's pregnancy announcement was captioned: 'Nothing is ever really lost. welcome back,' alongside an angel and heart emoji.
Her baby bump photo shoot comes a year after she revealed she suffered a miscarriage with MGK - whose real name is Richard Colson Baker.
She opened up about the experience in her 2023 book of poetry, Pretty Boys Are Poisonous.

Megan wrote about an ultrasound of a baby girl at 10 weeks and a day, per excerpts from People, and said: 'Maybe if you hadn't... maybe if i had...'
She continued, 'I want to hold your hand / hear your laugh, but now / I have to say / goodbye,' and later adds, 'I close my eyes and imagine holding you tight against my chest as they rip you from my insides.'
Megan and MGK had never discussed the pregnancy loss prior to the poetry book, however the Bloody Valentine hitmaker did appear to allude to it at the Billboard Music Awards in May 2022, dedicating his performance to Megan and adding, 'This is for our unborn child.'
Although the couple's engagement is back on and they seem to be more in love than ever, the Transformers actress hasn't been shy when it comes to the murky waters she and MGK have faced.
The lovebirds - who began dating in 2020 - have become known for their rocky and passionate relationship.
However, Megan isn't the only one to experience a turbulent romance with the rapper.
Since being thrust into the spotlight in 2012, MGK has been romantically linked to several women.
Now, as the musician prepares to have a second child, FEMAIL has laid bare the several ladies MGK has entertained before popping the question to Megan.
Megan Fox (June 2020 - present)
MGK and Megan met in Puerto Rico on set for their movie Midnight in the Switchgrass in March 2020.
The production was shut down after just two weeks of filming due to the coronavirus pandemic, but the pair continued getting to know each other on their own time.
A source told E!News: 'They've been hanging out a lot since their movie was shut down. The down time has been good for them... They are intrigued by each other and having a lot of fun.'
They were increasingly seen stepping out together and their public appearances coincided with a cryptic message posted to Instagram by Fox's husband, Brian Austin Green, on her birthday.
'Eventually butterflies get bored sitting on a flower for too long,' he wrote alongside a photo of a butterfly on a flower. 'They start feeling smothered. It's a great big world and they want to experience it.'
In May 2020, Beverly Hills 90210 alum Brian confirmed that he and Megan had split after nearly 10 years of marriage.
But speaking on his podcast, With Brian Austin Green, at the time there appeared to be no bad blood.
Brian, who confirmed the pair had been separated since December of 2019, revealed: 'She said, "I realized when I was out of the country working alone that I feel more like myself and I liked myself better during that experience and I think that might be something worth trying for me."
'I was shocked and I was upset about it, but I can't be upset at her because she didn't ask to feel that way. It wasn't a choice she made, that's the way she honestly felt.'
He also addressed her new rumored romance, adding: 'I've never met him, but Megan and I have talked about him.
'They're friends at this point, and from what she's expressed, he's a really nice, genuine guy, and I trust her judgment.'
That same month Megan took a starring role in MGK's music video for Bloody Valentine as the singer's domineering girlfriend.
By June, it seemed that MGK's relationship with the actress was official.
Tweeting a lyric from the song, MGK wrote: '"I'm calling you girlfriend, what the f**k." Life imitated art on that one.'
The newly-formed couple gave their first interview the following month when they appeared on the Give Them Lala... With Randall podcast.
Speaking about their connection, Megan said: 'I knew right away that he was what I call a twin flame.
'Instead of a soul mate, a twin flame is actually where a soul has ascended into a high enough level that it can be split into two different bodies at the same time.
'So we're actually two halves of the same soul, I think. And I said that to him almost immediately, because I felt it right away.'
The couple's relationship continued to blossom and they announced their engagement on Instagram in January 2022.
MGK posted a video of the daring ring alongside a caption that read: 'I know tradition is one ring, but I designed it with Stephen Webster to be two: the emerald (her birth stone) and the diamond (my birth stone) set on two magnetic bands of thorns that draw together as two halves of the same soul forming the obscure heart that is our love.'
And he later admitted that the ring was designed to hurt when it was taken off.
Bizarrely, the couple later revealed that they sealed the deal by drinking each other's blood - in what was just one of the odd features of their union.
In 2021, MGK also revealed that he wears a drop of Megan's blood on a necklace.
Previously sharing a photo of the unique transparent jewelry, featuring a red smudge, he captioned it: 'I wear your blood around my neck. My bloody valentine.'
Megan, who already shares sons Journey River, Bodhi Ransom and Noah Shannon with ex-husband Green, had even hinted at having children with MGK.
A source previously told Entertainment Tonight: 'They are so excited to spend the rest of their lives together, traveling the world, living in complete bliss and harmony, and potentially expanding their family one day.'
However, rumors of a split then put their romance on blast when Megan shared a cryptic post about 'dishonesty' - before completely deleting her account.
The Transformers star posted a video in which she appeared to burn a letter in a bonfire as well as a string of very raunchy mirror selfies while joined by a male pal.
Megan wrote in her caption, 'You can taste the dishonesty/it's all over your breath,' which are lyrics from Beyoncé's 2016 track, Pray You Catch Me.
Following fierce speculation from fans, MGK's guitarist Sophie Lloyd has since hit out at claims that she had played a role in the couple's previous alleged split.
The accomplished musician slammed the 'merciless' and 'disrespectful' rumors in a statement provided by her management team to Page Six.
'Sophie Lloyd is a professional, accomplished musician who has been needlessly dragged into the media based off of meritless accusations made by social media,' the statement began.
'Any suggestion that she has ever acted in an unprofessional manner or stepped out of her relationship are untrue.
'It's disrespectful to her as a female artist and poor journalism and social commentary to be reporting anything else.'
Sommer Ray (March 2020 - April 2020)
Influencer Sommer Ray, now 28, originally met MGK back in 2017 when they were both guests on Nick Cannon's MTV show Wild 'n Out.
But the pair were not linked romantically until 2020.
Sommer and MGK first fueled romance rumors after posting a series of snaps taken during a sun-soaked trip to the Bahamas with friends.
The pair got cozy while riding around in golf carts and playing on the beach with an adorable group of pigs.
And, just a short time later, they added fuel to the fire after putting on a very friendly display at his show at Rainbow Bar & Grill on the Sunset Strip, Los Angeles.
The rapper was seen taking her by the hand and leading her through the crowd before his set at the venue.
But MGK's relationship with Sommer, a fitness model and YouTube vlogger, came to an abrupt end.
In 2021, Sommer claimed that her former flame had 'kind of cheated' on her with Fox.
'If you look at the timeline of us dating, we were together in Puerto Rico,' Sommer told Logan Paul's podcast Impaulsive.
'I waited in the hotel the whole time when he's filming with her and I'm not really thinking anything of it. I thought she was older and like had kids and was married and stuff.'
She said that she began to suspect the self-described 'Blonde Don' had lost interest in her when she wasn't allowed on the set of his Bloody Valentine music video.
'I asked him why he didn't invite me to the music video and he said COVID restrictions,' the fitness-obsessed TikToker recalled.
'And then that music video came out and I was like, "Oh."'
'I'd probably do the same thing. Like I'd cheat on Colson with Megan Fox, like if it was the other way around I probably would be doing this,' Sommer stated.
Sommer also revealed: 'I dated Colson, never had sex with him... I make you wait at least three months. I have to make sure that you're someone good to me. I'm not going to make you a number if you're not good to me. So Colson just never passed the test.'
Breaking his silence on the split at the time, MGK tweeted: 'She came and picked all her stuff up on my birthday. Nice.'
But he seemed to have regrets shortly afterwards, adding: 'I shouldn't have tweeted personal business. Especially when the person is a great human and this tweet seems one-sided.'
Noah Cyrus (January 2020)
In 2020, MGK was linked to singer Noah Cyrus - who is nearly 10 years his junior.
The pair were spotted at a Grammys' afterparty where they posed together for pictures and held hands - and the PDA did not go unnoticed.
One source told US Weekly: 'MGK led the way and held his hand out behind him for Noah to grab.
'They stayed close to each other throughout the night. They clearly came as each other's dates.'
Noah, now 24, had also previously made a cameo in MGK's I Think I’m Okay music video in June 2019.
Despite the rumors, neither party ever confirmed a romance.
Chantel Jeffries (July 2019)
MGK was rumored to be dating DJ Chantel Jeffries, now 32, after being spotted on a series of romantic outings.
The pair first sparked dating rumors when they were spotted at the Peppermint Club in West Hollywood to watch comedian Dave Chapelle, before heading out for an intimate dinner.
And, just a short time later, they were at it again.
This time they were out celebrating at a Fourth of July get-together in LA wearing matching all-white outfits.
There were a handful of other rendezvous that followed but the alleged romance was short-lived.
Neither party ever officially confirmed their relationship.
Halsey (March 2017)
MGK was first linked to fellow musician Halsey when they were photographed on a beach together back in 2017.
In the picture, the electropop artist showcased her peachy posterior in a tan bikini, while the Bad Things rapper showed off his heavily tattooed torso by posing sans shirt.
But when photos of their sun-soaked getaway resurfaced some time later, Halsey, now 30, fiercely denied the dating rumors.
Responding to a since-deleted tweet of her at the beach with the rapper, she wrote: 'I'm not at the beach. I'm on tour. Alone. Slow news week I guess.'
She added: 'I'm not with anybody. That photo is two years old. Everyone mind they damn business [sic].'
The speculation only grew after Halsey was seen with Machine Gun Kelly in June 2018, just days before her breakup with G-Eazy, who she dated for nearly a year.
MGK was spotted patting her head as they smoked cigarettes on the street in Hollywood.
He later confirmed that they had never officially been in a relationship but claimed that he had been intimate with Halsey.
Speaking during an interview on The Breakfast Club, MGK was crudely asked: 'Did you smash Halsey?'
To which he replied: 'Absolutely.'
But there appeared to be no bad blood between the pair - and they reunited to release a single, Forget Me Too, in September 2020.
In the fiery video for the track, the duo played ex-lovers struggling to move on from their toxic relationship.
Amber Rose (April 2015 - June 2015)
MGK struck up a two-month fling with American model Amber Rose, now 41, in 2015.
The pair had been romantically linked after they were seen getting close at the airport in London in late April.
The Cleveland rapper confirmed they were seeing each other in May when he was asked on AOL Build if there were any tabloid rumors he had wanted to confirm.
The musician responded: 'What are you talking about – with Amber?'
'Ya'll know what's up!' he boasted.
Speaking about the pair's budding relationship at the time, MGK told radio station Hot 97: 'Everything was completely natural.
'It wasn't anything I had expected or had planned or anything like that. You know when the universe throws something your way or things happen, I'm one of those people that goes with it, and I don't fight it.'
Despite the short-lived nature of their relationship, the couple found time to spectate at the NBA Finals and even took a 'bae-cation.'
But their romance was not set to last with the pair reportedly splitting due to their busy schedules.
Rose, who had previously dated rapper Kanye West, told Perez Hilton on his podcast: 'We're not dating anymore. I think it was just a mutual agreement. We're still really good friends.'
Referencing her relationship with Wiz Khalifa, whom she was married to for three years, she said: 'I can't really support a man on tour anymore.
'I kind of did that already and I'm just not in a space where that's really what I want to do.
'I just need to focus on my son and my business. I'm dating other people and just having a good time.'
Emma Cannon (circa 2008)
Little is known about Machine Gun Kelly's first love Emma Cannon - whom he met when they were teenagers and dated before he entered the public eye.
The pair, who have not been pictured publicly together, reportedly met at a Blink-182 concert before the singer skyrocketed to fame with his first album Lace Up.
They welcomed daughter Casie Colson Baker in July 2009 in Cleveland, Ohio, when MGK was just 18 years old.
The Billboard Music Award winner often takes Casie to events with him as his plus one - but Cannon stays out of the limelight.
The former couple appear to still be amicable though, with MGK often speaking fondly of her on social media.
In one such example, on Mother's Day 2020, he tweeted: 'My daughter has an amazing mom. Big love to all the young moms out there killin' it.'
